Citizen Academy

Antioch Police Citizens Academy Application

Complete EVERY question unless stated as “optional,” or your application will be returned as incomplete.

Previous Academy Attendance:

Please include all other Citizens Academy or similar types of academies you have attended, including the name of the Academy and the year you participated in the Academy.

Legal:

The following questions are regarding arrests and convictions of any crime. If the records have been sealed, expunged, released or pardoned, do not list.
Include all misdemeanor and felonies. You do not have to include infractions - example, traffic ticket.

Class Attendance:

The Citizens Academy is an accelerated program with a full agenda each class session. Missing more than two classes will result in a significant gap in the education process of the academy. I understand that participants absent more than two days will not receive a certificate of graduation.

Antioch Police Citizens Academy Code of Conduct

  1. There will be no use of any tobacco products.
  2. Breaks will be limited to a maximum of fifteen (15) minutes and students are required to be in designated places when class resumes.
  3. Students are required to follow the instructions given by a classroom instructor or academy staff.
  4. Students are required to participate in all classroom/lab assignments and will not be excused from any activity without permission of the academy coordinators.
  5. Students are required to obey all signs and orders of the academy staff. Entrance into restricted areas without consent of the instructor is forbidden and may be cause for termination from the Citizens Academy at the discretion of the Chief of Police.
  6. Should a student become injured during the academy he/she must report the incident to the instructor or academy staff immediately.
  7. Possession or use of alcohol or other drugs are prohibited.
  8. Students should avoid obscene, vulgar, offensive or profane language at all times.
  9. Students should conduct themselves in a professional manner at all times.

Student Behavioral Contract

The purpose of this contract is to inform the above named student that he/she must comply with all provisions of the Law Enforcement Citizens Academy and to the specific terms set forth in this contract. The student understands that due to the nature of this academy there will be a zero tolerance rule in effect. Undesirable conduct such as horseplay or a violation of the student code of conduct will result in the immediate removal of the student from the academy. This contract in in effect for the safety of all students and instructors. This contract represents an agreement by the student that he/she received a copy of the Code of Conduct and the student agrees to adhere to this Code of Conduct at all times while enrolled in the Antioch Police Citizens Academy.

Dress Code

The academy has been developed to give each student the best possible learning experience. Therefore, it is necessary that dress and personal hygiene be developed and followed by each of the class members.

  1. Students must present themselves in a neat and well-groomed manner.
  2. No hats/caps, tank tops, open toe shoes, or shorts shall be worn in class. Students should dress business casual.
  3. With the exception of Class 6 (Weaponless Defense) no athletic wear (sweat pants) should be worn unless otherwise authorized by department or citizens academy staff.

ASSUMPTION OF RISK, RELEASE, AND WAIVER OF LIABILITY

Background Authorization

I understand that a criminal background and warrant check will be conducted by the Antioch Police Department as part of the application process. I hereby authorize any law enforcement agency to release to the Antioch Police Department any and all information, which said agencies have about me, for the limited purpose of aiding the Antioch Police Department in evaluating my eligibility for participation in the Citizens Academy. This authorization extends to any information which said agencies or any of them may have about me, whether public, personal, or confidential. I understand that I will not receive and am not entitled to know the contents of confidential reports received from these agencies. I hereby release, discharge, and agree to hold harmless the agencies, their agents and any person furnishing information from any and all liability arising out of furnishing and inspecting such documents and information.

I HAVE CAREFULLY READ THIS AGREEMENT, AND I UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND THE CITY, AND I SIGN IT OF MY OWN FREE WILL.
